UNDERSTANDING YOUR BENEFITS
Understanding Your Physical Therapy Benefits
Insurance coverage can vary significantly, even between two plans offered by the same insurance company. Your estimated responsibility may depend on several factors.
Copay
A fixed amount you may owe at each visit.
Deductible
The amount you may need to pay before your insurance begins sharing the cost of covered services.
Coinsurance
A percentage of the allowed amount that may remain your responsibility after your deductible is met.
Visit Limits
Some plans limit the number of physical therapy visits covered during a calendar year or benefit period.
Referral or Authorization
Certain plans may require a physician referral, prior authorization, or approval after a specified number of visits.
We can help verify the benefits reported by your insurer, but verification is not a guarantee of payment. Final coverage and claim decisions are made by your insurance company.

Physical therapy services are commonly eligible healthcare expenses. Acceptance of a specific HSA or FSA card depends on the card issuer and payment processor.
Payment Information
Payment is due at the time of service. We accept major credit and debit cards, and HSA or FSA cards may also be used when permitted by the card issuer.
Upon request, we can provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual plan.
