Prepaid Packages
Package pricing is available when visits are purchased together in advance.
12-Month Expiration
Package visits should be used within 12 months of the purchase date.
Non-Transferable
Packages are intended for the individual patient who purchased them and cannot be transferred to another person.
Standard Cancellation Policy Applies
Package visits remain subject to Oak City Physical Therapy's standard cancellation and no-show policies.
Using Insurance?
These packages are self-pay options and are not directly billed to insurance. If you plan to seek reimbursement from your insurance company, please ask us about documentation or superbill availability before purchasing. Reimbursement, when available, is determined by your insurance plan.
